Dear CESQG Participant:
Thank you for your interest in the proper disposal of hazardous waste from your business. Enclosed is information regarding the CESQG program.
The Regional Collection Center (RCC) will only accept hazardous waste from businesses determined to be Conditionally Exempt Small Quantity Generators (CESQGs). A CESQG is a business that generates no more than 220 pounds of hazardous waste per month and stores no more than 2,200 pounds on site at any one time.
In order to use the RCC, you must complete and return the Certification Form and Hazardous Products Listing sheet enclosed in the information packet to the address listed below or fax them back to us at 563-242-3611. Please make a copy of the Hazardous Products Listing sheet for your use of shipping papers.
Jason Bahnsen, Site Supervisor of the RCC program, will then contact you to verify the quantities of waste you have listed, give you a price for disposal and set up an appointment for you to bring out the waste. You may not bring waste in without an appointment. There is a minimum service fee of $10.00 for using the CESQG program.

CESQG CERTIFICATION FORM
In order to certify your generator status and dispose of your waste today, we are requesting that you sign the following statement.
I CERTIFY THAT I AM A HAZARDOUS WASTE GENERATOR AND BECAUSE OF THE SMALL VOLUME OF HAZARDOUS WASTE GENERATED AND/OR STORED, I QUALIFY FOR CONDITIONALLY EXEMPT SMALL QUANTITY GENERATOR STATUS.
I UNDERSTAND THAT TO QUALIFY FOR CESQG STATUS, I MUST MEET BOTH OF THE FOLLOWING CONDITIONS (AS DEFINED BY 40 CFR SECTION 261.5).
1) GENERATE LESS THAN 100 KG PER MONTH (220 LBS) OF HAZARDOUS WASTE PER MONTH, AND NEVER STORE MORE THAN 1,000 KG (2,200 LBS) ON SITE AT ANY TIME.
2) GENERATE LESS THAN 1 KG PER MONTH (2.2 LBS) OF ACUTE HAZARDOUS WASTE, AND NEVER STORE MORE THAN THIS AMOUNT ON SITE AT ANY ONE TIME.
I FURTHER UNDERSTAND THAT IF, IN THE FUTURE, I EXCEED THE QUANTITY LIMITATIONS DESCRIBED ABOVE, I WILL BECOME SUBJECT TO ADDITIONAL REGULATION AS A HAZARDOUS WASTE GENERATOR AND WILL NO LONGER BE ELIGIBLE TO PARTICIPATE IN THIS TYPE OF COLLECTION PROGRAM. ALSO, I UNDERSTAND THAT RCC PERSONNEL HAVE THE OPTION OF VISITING MY BUSINESS TO ASSESS MY HAZARDOUS WASTE, SHOULD ANY QUESTIONS ARISE.
